Current treatment strategy of acute promyelocytic leukemia

摘要: Acute promyelocytic leukemia (APL) is a unique subtype of acute myeloid leukemia (AML). The prognosis of APL has changed from the worst among the AMLs to currently the best. The application of all- retinoic acid (ATRA) in the induction therapy of APL decreases the high mortality of newly diagnosed patients, thereby significantly improving the response rate. ATRA combined with anthracycline-based chemotherapy is the current standard treatment, and for high-risk patients, high doses cytarabine have a beneficial effect on relapse prevention. In recent years, the indications of arsenic trioxide (ATO) therapy for APL have been extended from the salvage therapy for relapse patients to the first-line treatment of APL. The introduction of both ATRA and ATO represents great achievements in translational medicine. In this review article, we discuss the therapeutic strategies for this disease, including the initial approaches to newly diagnosed patients, prevention, and treatment of side effects and relapse to ensure the best and timely treatment for each newly diagnosed APL patient.

Adjuvant treatment strategy after curative resection for hepatocellular carcinoma

摘要: Hepatic resection represents the first-line treatment for patients with resectable hepatocellular carcinoma (HCC). However, the 5-year recurrence rates of HCC after surgery have been reported to range from 50% to 70%. In this review, we evaluated the available evidence for the efficiency of adjuvant treatments to prevent HCC recurrence after curative liver resection. Antiviral therapy has potential advantages in terms of reducing the recurrence rate and improving the overall survival (OS) and/or disease-free survival of patients with hepatitis-related HCC. Postoperative adjuvant transarterial chemoembolization can significantly reduce the intrahepatic recurrence rate and improve OS, especially for patients with a high risk of recurrence. The efficacy of molecular targeted drugs as an adjuvant therapy deserves further study. Adjuvant adoptive immunotherapy can significantly improve the clinical prognosis in the early stage. Randomized controlled trial (RCT) studies evaluating adjuvant immune checkpoint inhibitors are ongoing, and the results are highly expected. Adjuvant hepatic artery infusion chemotherapy might be beneficial in patients with vascular invasion. Huaier granule, a traditional Chinese medicine, has been proved to be effective in prolonging the recurrence-free survival and reducing extrahepatic recurrence. The efficiency of other adjuvant treatments needs to be further confirmed by large RCT studies.

Decentralized wastewater treatment technologies and management in Chinese villages

摘要: Problems treating sewage in rural areas in China have attracted increasing attention in recent years. However, only 6% of rural areas had spray drainage and wastewater treatment systems by the end of 2010. It is necessary to assess the technologies currently used so that sewage treatment in rural areas can be developed appropriately, to provide reliable and affordable wastewater treatment, in these areas. We evaluated the sewage treatment technologies currently used in rural areas by conducting case studies on 62 facilities that had each been operating for at least one year. Our study was carried out between 2009 and 2011, and the aim was to assess the situation during that period and assess any problems involved in decentralized sewage treatment in villages. We found that decentralized sewage treatment is the most popular wastewater treatment method in villages, and that the most common primary treatment technology used in rural areas is the septic tank. Our investigation highlights the need to establish standards for assessing effluent quality, including a range of target pollutants. Our results also show that effluents should be reused to meet the local environmental requirements in different areas, especially in ecologically sensitive areas.

Rare tumors: a blue ocean of investigation

摘要: Advances in novel drugs, therapies, and genetic techniques have revolutionized the diagnosis and treatment of cancers, substantially improving cancer patients’ prognosis. Although rare tumors account for a non-negligible number, the practice of precision medicine and development of novel therapies are largely hampered by many obstacles. Their low incidence and drastic regional disparities result in the difficulty of informative evidence-based diagnosis and subtyping. Sample exhaustion due to difficulty in diagnosis also leads to a lack of recommended therapeutic strategies in clinical guidelines, insufficient biomarkers for prognosis/efficacy, and inability to identify potential novel therapies in clinical trials. Herein, by reviewing the epidemiological data of Chinese solid tumors and publications defining rare tumors in other areas, we proposed a definition of rare tumor in China, including 515 tumor types with incidences of less than 2.5/100 000 per year. We also summarized the current diagnosis process, treatment recommendations, and global developmental progress of targeted drugs and immunotherapy agents on the status quo. Lastly, we pinpointed the current recommendation chance for patients with rare tumors to be involved in a clinical trial by NCCN. With this informative report, we aimed to raise awareness on the importance of rare tumor investigations and guarantee a bright future for rare tumor patients.

Treatment of advanced non-small cell lung cancer with driver mutations: current applications and future

摘要: With the improved understanding of driver mutations in non-small cell lung cancer (NSCLC), expanding the targeted therapeutic options improved the survival and safety. However, responses to these agents are commonly temporary and incomplete. Moreover, even patients with the same oncogenic driver gene can respond diversely to the same agent. Furthermore, the therapeutic role of immune-checkpoint inhibitors (ICIs) in oncogene-driven NSCLC remains unclear. Therefore, this review aimed to classify the management of NSCLC with driver mutations based on the gene subtype, concomitant mutation, and dynamic alternation. Then, we provide an overview of the resistant mechanism of target therapy occurring in targeted alternations (“target-dependent resistance”) and in the parallel and downstream pathways (“target-independent resistance”). Thirdly, we discuss the effectiveness of ICIs for NSCLC with driver mutations and the combined therapeutic approaches that might reverse the immunosuppressive tumor immune microenvironment. Finally, we listed the emerging treatment strategies for the new oncogenic alternations, and proposed the perspective of NSCLC with driver mutations. This review will guide clinicians to design tailored treatments for NSCLC with driver mutations.

A novel strategy for gas mitigation during swine manure odour treatment using seaweed and a microbial

摘要: Comprehensive mitigation of gas emissions from swine manure was investigated. Additives addition for mitigation of gas from the manure has been developed. Sargassum horneri, seaweed masking strategy controlled gas by 90%-100%. Immediate reduction in emitted gas and improving air quality has been determined. Microbial consortium with seaweed completely controlled gas emissions by 100%. Gas emissions from swine farms have an impact on air quality in the Republic of Korea. Swine manure stored in deep pits for a long time is a major source of harmful gas emissions. Therefore, we evaluated the mitigation of emissions of ammonia (NH3), hydrogen sulfide (H2S) and amine gases from swine manure with biological products such as seaweed (Sargassum horneri) and a microbial consortium (Bacillus subtilis (1.2 × 109 CFU/mL), Thiobacillus sp. (1.0 × 1010 CFU/mL) and Saccharomyces cerevisiae (2.0 × 109 CFU/mL)) used as additives due to their promising benefits for nutrient cycling. Overall, seaweed powder masking over two days provided notable control of over 98%-100% of the gas emissions. Furthermore, significant control of gas emissions was especially pronounced when seaweed powder masking along with a microbial consortium was applied, resulting in a gas reduction rate of 100% for NH3, amines and H2S over 10 days of treatment. The results also suggested that seaweed powder masking and a microbial consortium used in combination to reduce the gas emissions from swine manure reduced odour compared with that observed when the two additives were used alone. Without the consortium, seaweed decreased total volatile fatty acid (VFA) production. The proposed novel method of masking with a microbial consortium is promising for mitigating hazardous gases, simple, and environmentally beneficial. More research is warranted to determine the mechanisms underlying the seaweed and substrate interactions.

人工智能辅助诊疗发展现状与战略研究

摘要:

医疗健康数据的融合、开放共享,利用人工智能对碎片化医学信息进行整理分析,对医疗诊断过程提供辅助,可改善医疗健康服务,促进政府决策合理化,缓解医疗卫生资源配置不均衡问题。本文选取健康医疗信息人机交互、数据智能中的语义理解与医学影像分析等方面,简要阐述了人工智能在辅助诊疗问题上的发展方向与现状,讨论了智能诊疗技术发展与应用的问题与挑战:一是医学信息的标准化表征和结构化整合构建术语标准是关键;二是利用海量医学知识,构建多模态数据采集分析与结构化知识推理相结合的智能诊疗模型是重要的影像智能发展点。建议建立国家级的健康医疗开放大数据云平台,开辟新的数据、信息整合、知识发现及服务市场;构建医疗健康信息的一些基础行业标准,加强国产高端医疗器械的研发力度,推动智能化医疗器械和智能可穿戴式设备的研发,引导产业在人工智能与高端医疗器械的结合上开辟新的市场。

Seasonal and treatment-process variations in invertebrates in drinking water treatment plants

摘要: Abstract • Seasonal and treatment-process variations in invertebrates in a DWTP were analyzed. • The propagation and leakage of invertebrates in BAC filter were the most serious. • Invertebrates can survive and reproduce in chlorine disinfected clear water tanks. • Proportions of endogenous invertebrates increased along the treatment process. Problems associated with excessive propagation and leakage of invertebrates in drinking water have received increasing attention in recent years. We performed a monthly survey of invertebrate abundance and taxa in the effluent of each treatment stage in a drinking water treatment plant between May 2015 and April 2016 and analyzed seasonal and treatment-process variations in invertebrates. The results showed that invertebrate abundances in raw water, effluent of the biological activated carbon (BAC) filter, and finished water significantly correlated with water temperature, whereas no correlation was observed between water temperature and invertebrate abundance in the effluents of the sedimentation tank and sand filter. The dominant taxa in the effluent of each treatment stage were rotifers, nematodes, and crustaceans. The sedimentation tank could efficiently remove invertebrates with an annual average removal rate of 92%. The propagation and leakage of invertebrates occurred in the sand and BAC filters but more seriously in the latter. The average reproduction rate in the BAC filter was 268.8% with rotifers as the taxon that leaked the most. Invertebrate survival and reproduction were also observed in the chlorine-disinfected clean water reservoir with an average reproduction rate of 41.9%. Owing to differences in chlorine resistance, the reproduction ability of the dominant taxa was in the order nematodes>crustaceans>rotifers. The proportion of endogenous invertebrates gradually increased along the treatment process. The average proportion of endogenous invertebrates in the finished water was higher than 79.0%. Our findings suggested that waterworks should pay more attention to endogenous invertebrate growth.

Comparative genotoxicity of water processed by three drinking water treatment plants with different watertreatment procedures

摘要: Genotoxicity of substances is unknown in the water after treatment processes. Genotoxicity decreased by activated carbon treatment but increased by chlorination. Halogenated hydrocarbons and aromatic compounds contribute to genotoxicity. Genotoxicity was assessed by umu test; acute and chronic toxicity by ECOSAR. Inconsistent results confirmed that genotoxicity cannot be assessed by ECOSAR. Advanced water treatment is commonly used to remove micropollutants such as pesticides, endocrine disrupting chemicals, and disinfection byproducts in modern drinking water treatment plants. However, little attention has been paid to the changes in the genotoxicity of substances remaining in the water following the different water treatment processes. In this study, samples were collected from three drinking water treatment plants with different treatment processes. The treated water from each process was analyzed and compared for genotoxicity and the formation of organic compounds. The genotoxicity was evaluated by an umu test, and the acute and chronic toxicity was analyzed through Ecological Structure- Activity Relationship (ECOSAR). The results of the umu test indicated that biological activated carbon reduced the genotoxicity by 38%, 77%, and 46% in the three drinking water treatment plants, respectively, while chlorination increased the genotoxicity. Gas chromatograph-mass spectrometry analysis revealed that halogenated hydrocarbons and aromatic compounds were major contributors to genotoxicity. The results of ECOSAR were not consistent with those of the umu test. Therefore, we conclude that genotoxicity cannot be determined using ECOSAR .

Equipment–process–strategy integration for sustainable machining: a review

摘要: Although the manufacturing industry has improved the quality of processing, optimization and upgrading must be performed to meet the requirements of global sustainable development. Sustainable production is considered to be a favorable strategy for achieving machining upgrades characterized by high quality, high efficiency, energy savings, and emission reduction. Sustainable production has aroused widespread interest, but only a few scholars have studied the sustainability of machining from multiple dimensions. The sustainability of machining must be investigated multidimensionally and accurately. Thus, this study explores the sustainability of machining from the aspects of equipment, process, and strategy. In particular, the equipment, process, and strategy of sustainable machining are systematically analyzed and integrated into a research framework. Then, this study analyzes sustainable machining-oriented machining equipment from the aspects of machine tools, cutting tools, and materials such as cutting fluid. Machining processes are explored as important links of sustainable machining from the aspects of dry cutting, microlubrication, microcutting, low-temperature cutting, and multidirectional cutting. The strategies for sustainable machining are also analyzed from the aspects of energy-saving control, machining simulation, and process optimization of machine tools. Finally, opportunities and challenges, including policies and regulations toward sustainable machining, are discussed. This study is expected to offer prospects for sustainable machining development and strategies for implementing sustainable machining.
